1:1 Nutrition & Health Coaching Agreement and Liability Waiver

This Agreement and Liability Waiver (“Agreement”) is entered into between the undersigned participant (“Client”) and the Center for Aging Resources and Enrichment (“CARE”), regarding participation in 1:1 nutrition and/or health coaching services provided by CARE.

Nature of Services

CARE provides individualized coaching services focused on general wellness, lifestyle, and nutrition education. These services are intended to support personal goals and are not a substitute for medical care.

Client Acknowledgment

• Coaching services are not intended to diagnose, treat, cure, or prevent any disease.
• Any changes to diet, lifestyle, or medications should be discussed with the Client’s primary healthcare provider.
• The Client assumes full responsibility for decisions and actions taken based on coaching sessions.

Voluntary Participation and Assumption of Risk

The Client voluntarily elects to participate in coaching services and understands that participation involves lifestyle modifications that may carry risks. The Client assumes full responsibility for any known or unknown risks.

Confidentiality

CARE will protect the privacy and confidentiality of all information shared during sessions, in compliance with applicable laws and ethical standards.

Release of Liability

The Client releases, discharges, and holds harmless CARE from any and all claims, liabilities, or damages arising from participation.
