Home Evaluation Form

An elaborate 45 minute to 1 hour assessment completed by Occupational Therapists for older adults of living spaces & environment’s in the home. Assessment of any other specific client concerns related to safety and fall risks will be provided. Upon completion of the assessment, our team of Occupational Therapists will provide a detailed report of their findings and provide suggestions for a more safe living space related to your own environment to decrease your chance of falling & live more safely.
